Job Description

TITLE: Data Entry Support Clerk

Hours:            25 hours per week

GENERAL DESCRIPTION :

Under the direct supervision of nursing supervisors, the data entry support clerk will gather and enter all assessment data and consumer information into reporting program per EOEA requirements.

QUALIFICATIONS:

Knowledge of office procedures and practices required. Knowledge of Windows  operating system.   Ability to prepare and enter computer data effectively and in a timely manner. Ability to establish and maintain a good working relationship with staff and elderly clients. Proper  telephone etiquette required.

DUTIES AND RESPONSIBILITIES:

Enter data into computer system per EOEA and MassHealth program requirements.

Provide clerical support to medical secretary

Communicate issues and concerns within the nursing department

Provide supervisor with weekly updates of workload status

Perform any other duties assigned by supervisor

Job Title :                     Foster Grandparent Program -Administrative Assistant

PART TIME 20 hrs

The Foster Grandparent Program provides volunteer opportunities for income eligible people aged 55 and over.  Foster Grandparent volunteers are placed in elementary schools, nursery schools, special needs schools, youth centers and hospitals where they have the wonderful opportunity to help with children's growth and development through games, reading books, crafts or just having a one-on-one conversation with the child.

General Description :        

The Foster Grandparent Program- Administrative Assistant under the Project Director shall perform general secretarial and bookkeeping duties.

Qualifications :

*       Ability to establish/ maintain good working relations with sites & volunteers  

*       Knowledge of computer programs inc Microsoft Word, Excel, Print Shop

*       Knowledge of office & bookkeeping practices and procedures.

*       Ability to work independently, good organizational, planning & record keeping skills with attention to detail.

*       Sensitivity to elderly and to the confidentiality of information.

Duties and Responsibilities

*       Perform various clerical/secretarial duties

*       Answer the telephone & provide program information

*       Monitor & maintain accurate program records, time sheets/payroll 

*       Maintain volunteer and site records on in house data system.

*      Assist with the bimonthly program newsletter

*      Assist Foster Grandparent Director in planning volunteer in-services, annual         recognition parties and social trips.  Attend Advisory meetings.

*      Prepare & coordinate mailings of volunteer time sheets etc. Record receipts of funds for submission to the fiscal department.

*      Perform other duties as may be required.

Title: Case Manager-Full Time

General Description:

The Case Manager within the framework of the Home Care Regulations, assesses and identifies client's needs and strengths, develops, coordinates, and follows up service plan based on these needs.  The Case Manager achieves the goal of working with the total person by developing an ongoing helping relationship.  The Case Manager maintains accurate and up-to-date client files and promotes personal and professional development through supervision and training opportunities.

Qualifications:

B.A. from an accredited college or university, preferable in social services, or commensurate work experience in human services, or training in the field of gerontology.

Experience working with the elderly.

Knowledge of local community resources.

Driver's license and car.

Communication skills and the ability to relate well with others.

Writing concise, professional, problem-oriented narratives.

Ability to advocate for others.

Fluency in English and Portuguese

Duties & Responsibilities:

Assess the applicant-recipient's needs for supportive services through a home visit evaluation of his/her social, physical, emotional, and environmental circumstances as needed.

Determine financial eligibility of clients for Home Care services and assess eligibility for SSI, Medicaid, etc.

Develop service plan and supportive networks through contact with client, those involved with client (family members, physician, clergy, etc.)  and consultation with supervisor.

Re-evaluate and revise service plans on a regular basis to meet new goals and/or changing needs.

Interpret agency policies, procedures, services to clients and the public.  Inform client of his/her rights to obtaining and maintaining Home Care services.

Maintain client case records in an accurate, complete and confidential manner including all required forms.

Advocate on behalf of applicant/recipient (when appropriate) to ensure that needs are being met.

Attend staff meetings and other such meetings as indicated.

Provide information and referral services to applicants.

Attend education workshops, classes, conferences to increase knowledge and awareness of aging process, with approval of supervisor.

Other duties and responsibilities including special projects which are reasonable and related to Home Care goals and caseworker job functions may be assigned.

Meet on a regular basis (not less than once a month) with supervisor.

Provide office coverage/support for co-workers on a regular schedule.

Cooperate with staff in implementing service plans.

Evaluate homemaker performance on a regular basis.
