Home Care Services/Care Management

When an illness or disability strikes, or when your ability to perform the normal activities of daily living diminishes, there may be the need for a variety of Home Care Services that will allow you to remain at home or return home. Coastline's home care service area includes: New Bedford, Dartmouth, Acushnet, Fairhaven, Mattapoisett, Marion, Rochester, and Gosnold. WHAT WILL HOME CARE NURSES AND CARE MANAGERS PROVIDE?

Home Care Services may range from brief to ongoing care, daily care or a few times a week.

  • We will come to your home and help you determine your home care needs by talking with you and if you desire, your family members.
  • We will answer any questions you may have so you are able to make an informed decision.
  • We will, together, develop a plan of care which will provide the services to meet your needs, enabling you to remain at home.
  • We will arrange the services with contracted providers, determine the costs beforehand, and monitor your care on a regular basis to ensure that you are:
    • receiving reliable, quality care,
    • satisfied with the services and that the providers are meeting your needs.
  • We also provide a Respite Program for the relief of the primary caregiver who is experiencing stress with the care of the elder.
  • Those interested in receiving a home assessment by a qualified case manager may call directly or visit the Agency between the hours of 8am and 5pm, or have a health care professional call or fax a referral.
  • To be eligible for the state subsidized Home Care Program an elder must:
    • Be 60 years of age or older.
    • Need help with at least six tasks of daily living including at least one critical unmet need. Critical needs include bathing, dressing, eating, toileting, meal preparation, food shopping, transportation for dialysis, chemotherapy, and radiation only.
    • Pay a co-payment based on income from all sources.
    • Must live in a home setting .
    To be eligible for the Respite Program an elder must:
    • Be 60 years of age or older or be diagnosed with Alzheimer's disease.
    • Have an unpaid caregiver who is providing daily care and experiencing stress.
    • Be severely functionally impaired.
    To be eligible for Home Delivered Meals an elder must:
    • Be 60 years of age or older
    • Be homebound and/or unable to prepare a meal.
    • Be without other assistance in the home.

HOME CARE SERVICE OPTIONS:

  • Adult Day Health : Services provided by an Adult Day Health program approved for operation by the Division of Medical Assistance and operating in accordance with 130 CMR 404.000 et seq. ADH services provide health care, supervision, restorative services, and socialization.
  • Chore : Services to help clients maintain their homes and/or to correct or prevent environmental defects that are hazardous to a client's health and safety. Heavy chores include washing floors and walls, defrosting freezers, cleaning ovens, cleaning attics and basements to remove fire and health hazards.
  • Dementia Day Care : Services provided by a Dementia Day Care Program operating in accordance with Dementia Day Care Standards issued by Elder Affairs. Dementia Day Care provides a structured, secure environment for individuals with cognitive disabilities to maximize the individual's functional capacity, reduce agitation, disruptive behavior, the need for psychoactive medication, and to enhance cognitive functioning.
  • Emergency Shelter : Services designed to provide temporary (for no more than 14 calendar days in a six month period) overnight shelter for a client and the client's household who are without a home.
  • Enhanced Personal Emergency Response System : A service which has the capacity to program a PERS console unit so that messages from family members or friends may be pre-recorded from a remote location and transmitted to the client at established intervals. It is designed to cue clients for medication reminders, appointments, and "check-in calls".
  • Home Delivered Meals : The provision of well balanced meals which meet Elder Affairs' Nutrition Standards and the client's dietary needs, delivered to clients who are unable to prepare nutritionally adequate meals or attend a congregate meal site.
  • Home Health Services : Those services defined in DMA regulations in 130 CMR 403.000 which include skilled nursing care; physical, occupational, and speech therapy; and home health aide.
  • Homemaker/Personal Care Services : Services provided by a trained/skilled individual to meet a client's ADL and IADL needs. Agencies must comply with the standards set forth by the Executive Office of Elder Affairs.
  • Laundry Service : Services provided by commercial laundry companies. Activities include: pick-up, washing, drying, folding, wrapping, and returning of laundry.
  • Medication Dispensing System : This service is designed to assist elders with medication compliance problems. The system is programmed to deliver the correct dosage medication as appropriate. This service shall only be authorized when a caregiver can demonstrate the ability to pre-fill medications and monitor the system.
  • Personal Emergency Response System : A medical communications alerting system that allows a client experiencing a medical emergency at home to activate electronic components which transmit a coded signal via digital equipment over telephone lines to a central monitoring station. The central monitoring station is staffed 24 hours a day, seven days a week by trained attendants who receive and process the emergency call and ensure the timely notification needed to dispatch appropriate individuals and/or emergency services to the person in need.
  • Short Term Care in a Skilled Nursing Facility : This service provides skilled and routine nursing care; rehabilitative services such as physical, occupational, and speech therapy; and assistance with activities of daily living such as eating, dressing, toileting, and bathing. This 24 hours a day/7 days a week care is provided to elders in need of this short term care.
    A skilled nursing facility must be licensed by the Department of Public Health and operate in accordance with 105 CMR 150.00.
  • Short Term Care in an Assisted Living Facility : This service provides room, board, and personal care services by an entity certified by the Executive Office of Elder Affairs.
  • Supportive Day Programs : Individualized programs of social activities provided to clients who require daytime supervision at sites other than their homes. Activities include: assistance with walking, grooming, and eating; provision of one meal and two snacks per day; planned recreational and social activities suited to the needs of the participants and designed to encourage physical and mental exercise and stimulate social interaction.
  • Supportive Home Day Care Aide : Services provided to clients with emotional or behavioral problems to assist with activities of daily living and instrumental activities of daily living.
  • Translation/Interpreting Services : Translation/interpreting provided by skilled individuals to clients in need of such assistance in order to receive services.
  • Transportation : This service provides transportation to and from medical appointments.
  • Wanderer Location Service : A program designed to register people with dementia related illness who are at risk of wandering and becoming lost. All registered patients are assigned a unique code number that is kept in a central registry. The service works with local providers to return the patient to a safe location and notify caregivers to provide follow up and support to minimize the likelihood of further wandering incidents.
